The answer
The Issues view answers a different question from an error dashboard. Reliability errors show whether execution broke. Behavior issues show whether the agent's actions and results failed the user even when the request completed.
The production taxonomy covers looping and stuck behavior, drifting, user frustration, abandonment, result quality, hallucination and trust, and safety and policy. MCP views add failed calls, repeated-call loops, telemetry gaps, latency, and outcome coverage.
Each issue includes an explanation and affected sessions rather than only a count. Inspect the technical detection details and where the pattern fires before changing the agent.
